How can I get a copy of my Military Discharge DD Form 214, Report of Separation (or equivalent)?

Veterans and U.S. military service members and their next-of-kin can obtain free copies of their DD Form 214 (Report of Separation) and other records several ways:


    National Personnel Records Center
    Military Records
    9700 Page Boulevard
    St. Louis, MO 63132-5100

Veterans and U.S. military service members can also order their Military Discharge (DD214) through the Los Angeles County Registrar-Recorder/County Clerk office by faxing their application for DD214, Military Discharge to (562) 864-1250 or mailing their request to:

The request must be accompanied by a notarized certificate of identity sworn under the penalty of perjury that the requestor is an authorized person as noted above and a photocopy of the requestor's photo identification. I am a homeless veteran in need of help, who can I call?

  • You can call the National Call Center for Homeless Veterans at: 1-877-4AID VET (1-877-424-3838). The Veterans and U.S. Military Resources webpage provides an array of local agencies that are designed to help homeless veterans and U.S. military service members. For information about programs and services for homeless veterans, please click on Housing to access information on the various agencies that can help.

I am homeless and have tickets that I am unable to pay for, where can I go for help?

If you have had a history of homelessness, you may be able to clear tickets and warrants for minor offenses such as traffic tickets, infractions, and various minor misdemeanors including jaywalking, riding the metro without a fare, sleeping in public places, illegal use of shopping carts, possession of an open container of alcohol in public, obstructing traffic on the sidewalk, sleeping on a bus bench, as well as any warrants that arise for failure to resolve these offenses handled through the Homeless Court. You may also contact your local County Veterans Service Office for assistance at 213-744-4825.

 I am a veteran and I'm having trouble finding employment, are there employment resources especially for veterans?

  • There are several employment resources specifically for veterans and U.S. military service members. Just click on Employment to find a list of employment resources such as the Career One-Stop Veterans Center who have specially-trained staff to ensure service members of the U.S. Armed Forces receive maximum employment and training opportunities.


    Veteran and U.S. military service members are granted priority status through the CalJOBS labor exchange system which includes:

    1- 24-hour "Veterans Only" hold on all new job orders received.
    2- Placement of qualified veterans' resumes before non-veterans' resumes on job order screens that are viewed by EDD staff or prospective employers.

    Another resource accessible through this webpage is Hire A Hero USA, which is a non-profit charity networking site that provides information about military-friendly employers.

What application(s) do I need to file to request a correction of my military service record? 

Veterans or U.S. military service members need to file a DD Form 293, Application for the Review of Discharge or Dismissal from the Armed Forces of the United States, to the Discharge Review Board of the military service branch they served to request a review.

If the discharge was more than 15 years ago, service members may submit a DD Form 149, Application for Correction of Military Records, to the Board of Correction of the military service branch they served to request a review.

Where can I find emergency shelters?

Faith-based organizations such as Salvation Army and Volunteers of America may have a variety of programs to assist you. You may also dial 2-1-1 for additional information.

Where can I find rental assistance?

You can find organizations that provide rental assistance at Rent Assistance, or by calling the National Call Center for Homeless Veterans at: 1-877-4AID VET (1-877-424-3838). Vets On The Move, is a public benefit charitable non-profit foundation based in Los Angeles. It provides housing for veterans and U.S. military service members and their families through many options; including low-income tenancy, Veterans Administration Supportive Housing (VASH) and Section 8 housing solutions. Families are provided affordable tenancy and single persons are provided shared living environments.
