SSI Supportive Services


Supportive services are provided to assist General Relief (GR) participants to overcome barriers in completing the Supplemental Security Income (SSI) process.  Supportive services include referrals to medical and mental health care, as well as, issuances that can assist the participant in overcoming barriers to comply with any Supplemental Security Income and Medi-Cal Advocacy Program (SSIMAP) related requirements.


Key Points

  • The need for mental health services is evaluated during the GR application process. In addition, a participant can request an evaluation at any time.
  • Medical and mental health service providers become sources of records in support of the SSI application or during the SSI appeals stages.
  • Transportation can be authorized by the SSIMAP Advocate or Eligibility Worker for any medical or SSI-related appointment.
  • Additional issuances by the SSIMAP Advocate may be permitted for:
    • Clothing;
    • Showers; and
    • Haircuts.
    • Taxi vouchers are available from the State's Disability Determination Service (DDS) to provide door-to-door transportation to and from SSI medical appointments scheduled by DDS.
  • As public transit providers have special fares for disabled customers, this information is shared with the participants.

Mental Health

  • Participants identified as potentially in need of mental health services are referred for a mental health evaluation. Participants can also receive a mental health evaluation if requested.
  • As part of the GR application process, a participant's employability (or ability to work) is determined.
  • If the assessment is for a mental health disability (Needs Special Assistance - NSA), the mental health professional makes this evaluation on the ABP 2012, Referral for Mental Health Services.
  • Clinicians from the Department of Mental Health (DMH) conduct the evaluations in all District offices except Civic Center, which is served by Community and Senior Services' Adult Protective Services (APS) Workers.
  • If a DMH Clinician (or APS Worker) is unavailable to conduct the mental health evaluation the same day the participant is in the office, the SSIMAP Advocate will conduct the mental health evaluation. As this evaluation may result in a temporary unemployable status, the participant will be scheduled a future appointment with a DMH Clinician (or APS Worker).
  • If treatment is recommended, an initial appointment with a mental health provider is scheduled by the DMH staff.  

Physical Health

  • Participants identified as potentially in need of physical health services are referred for a physical health evaluation.
  • As part of the GR application process, a participant's employability (or ability to work) is determined.
  • All GR participants are encouraged to apply for Medi-Cal. If a physical health assessment is needed, the participant will have their medical provider conduct the assessment.
  • If the assessment is for a physical health disability (Unemployable), the medical provider makes this evaluation on the ABP 1676-1, Physical Health Assessment for General Relief.

SSI Advocate Duties

At the initial interview, and at each subsequent contact, evaluates the need for supportive services.

  • Makes any needed referrals for medical or mental health care.
  • Arranges for transportation funds for any medical or SSI-related appointments.
  • Records all participant contact and SSIMAP case activity on the electronic case record.
  • Updates the electronic case record with new or changed information and documents all actions taken.

Results

Upon an evaluation that supportive services are needed:

  • A referral for medical or mental health care (or assessment, if needed) is completed.
  • Transportation was arranged, as needed.
  • All participant contact and SSIMAP case activity on the electronic case record is updated.
  • The electronic case record with new or changed information is documented.
