CHILD HEALTH & DISABILITY PREVENTION (CHDP) PROGRAM

Children with Medi-Cal (birth to age 21) or children without Medi-Cal (birth to age 19 years) in low to moderate income families are eligible for free immunization shots and health check-ups. Families can choose a CHDP approved private doctor, clinic or other health care provider.

What services are offered?
Based on age, a child is entitled to 15 or more exams per year. Services include regular complete health checkups and shots, diagnosis and treatment (including hospitalization) for conditions found during the health checkup, and prescription medicines.

Who can be covered?
Medi-Cal participants from birth through 20 years of age and low to moderate income (Up to 200% of the Federal Poverty Level (FPL) clients from birth through 18 years of age. Children in Headstart and State Preschool programs and child in Foster Care.

Is there an income limit?
Yes, Up to 200% of the Federal Poverty Level (FPL). See Chart below for further details.
To qualify for the program, the family's annual income must be BELOW the income limit specified for the family size. For example, for a family of two, the family's monthly income must be less than $2,200.

No.

Can CHDP services be received if there is other health insurance?
Sometimes. Ask your CHDP doctor.

How much does it cost?
No cost.

Is citizenship or legal residency required?
No.

Call your Medi-Cal Managed Care provider or a CHDP approved doctor and ask for a CHDP exam appointment.
